To get started, please call our office 770-410-7719.
You may get a jump start by completing the form(s) below – they will take approximately 10-15 minutes to complete. All information is confidential.
If your child has had a recent assessment completed by other health care professionals, including Speech Pathologist, Audiologist, ENT, Developmental Pediatrician, Neuropsychologist or school IEP, please provide copies so we are best able to understand your child’s needs.

Cancellation Policy –
We follow a strict Appointment and Cancellation Policy
Please review our Cancellation Policy below to avoid any unnecessary charges. (The complete Cancellation Policy is included in the Policy Service Agreement)
Please be aware of our Cancellation Policy to avoid any unnecessary charges and/or to maintain a weekly standing appointment on our Therapy Schedule.
We reserve the right to charge $50 as and for an appointment not canceled within 12 hours due to sickness or other circumstance.
We reserve the right to remove your weekly appointment from the Therapy Schedule if the appointment is canceled two times within a given month, for inconsistent therapy attendance or for any other reason at our discretion.
NO SHOW (Not attending an appointment without notice and/or proper cancellation):
Not Showing for a scheduled appointment will not be tolerated and will result in the removal from the Therapy Schedule (at our discretion). We do not re-schedule new Patient Evaluations if the first appointment was a NO SHOW.
IMPORTANT ! We do not hold appointments if your child is unable to attend Therapy consistently.
If you go out of town for more than one week of missed sessions or for any extended period, you may lose the appointment spot on our weekly Therapy Schedule. Upon your return, we will work with you to schedule therapy with the appointments we have available at that time. We do not guarantee your previous appointment day and times will be available.
If your child becomes sick the morning of your Therapy appointment we ask that you contact your Therapist immediately via text so they are aware. If your child goes to bed sick, please text your therapist the evening before your appointment. This helps us to prepare and to offer the appointment to families on our Waiting List.
In general, cancellations are best received by texting your Therapist directly but can also be canceled by calling our office and/or leaving a voice mail 770-410-7719.
Weekly Appointment(s) – Therapist Coverage
As a health insurance requirement for medically necessary therapy treatment, your child’s Therapy is guided by a Plan of Care.
In general, it is our intention that your child’s weekly therapy will be provided by the same Therapist(s) each week; however, when the Provider you normally work with is not available (i.e. unexpected sickness or for any reason), we will continue to provide therapy services under the Plan of Care with one of our other, qualified Therapist(s). Since canceling Therapy is not recommended and is not supported by the Plan of Care, we will never choose to cancel an appointment when we have a qualified Therapist available to provide therapy to your child.
The Therapy provided here is collaborative and our team diligently works together in therapy preparation to assure we provide superior care to the families we serve.
We realize an unexpected change of Therapist may feel a little different ! however, we assure you that your child’s therapy remains at the forefront and the covering Therapist is prepared to provide your child with a quality therapy session!
New !! Alternative Appointment Option for Speech Therapy:
Weekly Standing Therapy appointments: our experience, in addition to evidenced based research, has shown that consistent weekly therapy is essential for successful Speech Therapy. In order to maintain a spot on our weekly Therapy Schedule, we require strict adherence to our Cancellation and Attendance Policy.
Alternative option for Speech Therapy appointments:
We realize this environment may not work for all families and committing to weekly standing Therapy appointments may be difficult; therefore in order to help facilitate weekly appointments we have an alternative option available to allow for your child to continue weekly Speech Therapy (and limited options for Occupational Therapy).
We offer parents the opportunity to call or text our office on Mondays between the hours of 9:00 – 3:00 and request an appointment within the week. We can offer you an appointment based upon availability for the week. (TEXT 434-623-0005 or call 770-410-7719)
If this is a better option for you and you are interested, please call the office to obtain the phone number to text on Mondays. Texts received on Mondays to our Appointment Text Line will allow us to offer available appointments for that given week.
